This form MUST be filled out and signed within 78 hours prior to your skincare appointment, for safety precautions. It is preferred that you sign the form, then scan and email it back to Andrea, or you may bring it to your appointment. But it is crucial that you go over this consent form 3 days prior to the appointment to ensure that your skin is prepared for the treatment. If the necessary precautions are not taken, Andrea will not be able to perform the service, either chemical peel or waxing procedure, that you would like.
